Medicaid fraud is just any deliberate false claim to the medicaid program – billing for a no-show visit or up-coding a check-up counts. As you might expect, it’s a serious issue since it siphons taxpayer dollars, which is why it invites reviews from state medicaid agencies and federal agencies.

How common is it?

CMS tracks the national improper payment rate through its Payment Error Rate Measurement (PERM) program. In fiscal year 2024, the rate was 5.09%, which is equal to roughly $31 billion in federal payments that should not have gone out. Fraudsters draw from that pool.
